You are not getting QI. The possibility can be that you are playing with your own alts in private server. It is a measure against multi-clienting. Although other problems can cause this too but I am not aware of any.

I would say if youre playing on 2 accounts or more at same time
Close one of them
And if you arent then i would say close tori and wait like 10 minutes then it always works when it happens to me
