Depending on the auction or person you're buying it from then they have to have a specified date that the auction ends. If you have the highest bid when the auction ends, you will receive the item. If they DON'T send you the item when you are the highest bidder and it's the end date then you can report them and have it sorted out. Also, most bids have an auto-buy on them that lets you automatically purchase the item without bidding.
What Lume said is right when it comes to toribash and some other bidding.
In toribash, if the winning bid didn't send what was due, he must pay the auctioneer 40% of what is due to him.

In real life, it is not always the highest bidder who wins the auction. It depends on the auctioneer on who he wants to pick. Because for example when buying materials, others bid with low prices but also with low quality.
