Apart from what Albus said , Dq detection can sometimes fail when one or both of the players grabs the other. Usually happens in judo , especially if there's alot of collision and dm going on at the same time , resulting in lag , as Albus stated.

sometimes in aikido there are collision errors when someone touches outside of the ring, these can be temporary or last the entire match with no dqs

its a glitch with maxcontacts. it happens every once in a while if theres too much pressure on certain joints. other times like in twinswords there is a similar glitch that can prevent dq outside the ring for the rest of the match, simply because there were too many dms at one time.
